Take a Moment to Reflect

Before You Begin

This short, five question self-check is based on the Severity of Dependence Scale (SDS), a well established measure developed by Gossop and Colleagues (1995). The SDS has been gently adapted for people who are either thinking about making a change, or wanting to maintain the changes they've already made. The SDS is not a diagnosis or a test, just a quick reflection tool that helps you think about your relationship with substances.

Take a moment, answer honestly, and go with your first response. When you finish, you’ll see your results.

Note: This self-check has been validated for a range of substances. The word "drug" refers broadly to any substance, alcohol, medications, or other drugs.

Remember, this isn’t a diagnosis.

It's just a quick guide to help you understand where you’re at. Whether your score is low or high, The Wagon can help you take the next step in the direction you want.
