This short, five question self-check is based on the Severity of Dependence Scale (SDS), a well established measure developed by Gossop and Colleagues (1995). The SDS has been gently adapted for people who are either thinking about making a change, or wanting to maintain the changes they've already made. The SDS is not a diagnosis or a test, just a quick reflection tool that helps you think about your relationship with substances.
